4. Certification of PPS Specialty Board Exam (at least written exam)
5. Certificate of Residency
6. Accomplished Application Form
7. Application Letter addressed to Myrna B. Rosel, M.D. (Chairman)
8. Application Fee (P 500.00)
9. Board Certificate and PRC License
10. 2 Recommendation letters from 2 previous consultants (PPS Fellows)
11. Essay:
1. Why do you want to go on Pediatric Nephrology training?
2. How do you see yourself 5 years from now?
